Your Health Is Our #1 Priority
Emergencies
For life threatening conditions call 911 or go to the nearest emergency room. There is a physician on call 24/7 to help you with noncritical medical problems.
Appointments
  • In order to minimize your wait time, be punctual for appointments.
  • We need 24 hours advance notice when rescheduling or canceling your appointment. Failure to do so can result in additional charge.
  • New referrals, physicals and disability forms will be completed only after an office visit.
Prescriptions
  • To prevent complications, prescriptions refills will only be provided with regular follow-up visits.
  • For your safety, please do not ask us to diagnose illness or prescribe medication over the telephone.
Test Results
  • Please visit our website (www.mhaqmd.com) to view your test results.
  • Only patients with abnormal test results will receive a notification from our office.
Pay Your Balance
  • Co-pays, deductibles, form completion fees and any other outstanding balances should be paid promptly.
Health Information
  • Please update your medical history and contact information by visiting our website.
